This is an exciting opportunity for a dynamic, organised and values-led Project Manager to join our Senior Operations Team and play a pivotal role in shaping and delivering transformational primary care services across Grantham and Sleaford.
You will lead the delivery of a diverse portfolio of projects, supporting both the transformation of existing services and the implementation of new, innovative models of care. Working closely with clinical leaders, GP practices and system partners, you will ensure services are designed, mobilised and embedded successfully.
You will also play a key role in supporting our Primary Care Networks to deliver PCN DES contractual requirements and maximise shared income, while contributing to wider system collaboration across Lincolnshire.

Main duties of the job

Service Design
Turn new service proposals into design solutions by engaging with operational and clinical stakeholders to identify practical solutions to design and deliver new services.
Business Case Development
Using design solutions for new services to develop commercial business cases that demonstrate value for money for the Clinical Commissioner and sustainable income for K2 Healthcare. Including: Options and recommendations
Project Management
Development of clear and comprehensive implementation and change plans. End-to-end project delivery including:
Primary Care Network Directed Enhanced Service
Supporting practices to deliver the PCN contractual requirements and to meet PCN DES achievement targets to maximise shared income
Some operational management may be required on an interim basis and as part of transition to business as normal.
