The Project Manager will lead the design, development anddelivery of new and existing services across the Lincolnshire PCN Alliance,supporting system-wide priorities and the ongoing transformation of primarycare.
Working collaboratively with clinical, operational andsystem partners, the role will translate strategic priorities into practical,deliverable service models, ensuring alignment with commissioning requirementsand the PCNA business plan.
The initial focus of the role will be to lead thedevelopment and implementation of a new, county-wide weight management service,designed to support patients through accessible, community-based interventionsand multidisciplinary care. This will involve bringing together partners acrossprimary care, community services and wider system organisations to deliver acoordinated, outcomes-focused model that supports long-term behaviour changeand improved population health.
This is a fixed-term, full-time contract for 12 months with an anticipated start date as soon as possible. Due to service need, we are prioritising candidates who are able to start promptly, and this role may not be suitable for individuals with long notice periods.
Main duties of the job
The role will support the development andimplementation of new models of care, coordinating activity across multiplestakeholders to ensure effective planning, delivery and embedding of change.This includes working across organisational boundaries to enable collaboration,align priorities and support the development of the Alliance and its memberPCNs.
The postholder will provide projectleadership, coordination and oversight, ensuring that programmes are deliveredto a high standard, within agreed timescales, and that outcomes are achievedand sustained.
