Main duties of the job
As a Senior Individual Assessment and Commissioning Nurse, you will play a vital role in relation to completion of robust care needs assessments to help determine eligibility for NHS funding. For those who meet the criteria for NHS funding, you will work in partnership to develop a personalised care support plan to meet their assessed needs which support safe delivery of a high-quality, person-centred package of care either in their own home or within a care setting.

- Lead on assessment and case management
- Commission safe, effective, and tailored care packages
- Ensure care is delivered in a timely, person-centred, and outcome-focused way
- Work collaboratively with patients, families, providers, and multidisciplinary teams
- Navigate and apply national policy, guidance, and legislation
- Support safe and timely hospital discharges and transitions of care
- Contribute to performance targets, quality standards, and service improvement
Working pattern
One of the key benefits of this role is its excellent work-life balance:
- Monday to Friday
- 08:00 am - 16:00 or 09:00 - 17:00
- No weekends or bank holidays

- Nursing qualification or equivalent, for example Occupational Health, Physiotherapist, Social Worker etc with current registration.
- Evidence of CPD
